Privacy Policy

Effective Date: September 17, 2026  ·  Last Updated: September 17, 2026

AC Personal Care LLC ("AC Personal Care," "we," "us," or "our") respects your privacy. This Privacy Policy explains what information we collect through our website, acpersonalcare.com, how we use and share it, and the choices available to you.

1. Overview

This Privacy Policy applies to information collected through our website. It does not apply to information collected offline, such as during an in-home assessment, over the phone, or on paper intake forms — that information is instead covered by our Notice of Privacy Practices, since it typically relates to the health care services we provide.

By using our website, you agree to the collection and use of information as described in this policy.

2. Information We Collect

Information you provide to us

When you fill out our contact form to request services or ask a question, we collect the information you choose to submit, which may include your name, phone number, email address, and the contents of your message.

Information collected automatically

Like most websites, our web hosting provider may automatically log standard technical information when you visit — such as your general browser type, device type, and pages viewed — for security and site-reliability purposes. We do not currently use this information to build profiles about individual visitors.

Information from third-party services

Our website uses a small number of third-party services to operate:

  • Google Fonts — we load typefaces directly from Google's servers. Google may log the IP address of the device requesting a font file. See Google's Privacy Policy for details.
  • FormSubmit — our contact form is processed by FormSubmit, a third-party form-delivery service that forwards your submission to our email inbox at hello@acpersonalcare.com. FormSubmit does not receive any payment or health information from us, only the fields in the contact form itself. See FormSubmit's Privacy Policy.

3. How We Use Your Information

We use the information you submit through our website to:

  • Respond to your questions and requests for service;
  • Schedule an in-home assessment or follow up about care options;
  • Maintain the security and proper functioning of our website; and
  • Comply with applicable legal obligations.

We do not use website contact-form information for advertising or marketing profiling, and we do not sell it.

4. How We Share Your Information

We do not sell your personal information. We may share information you submit through our website with:

  • Service providers who help us operate the website and process contact-form submissions (see Section 2), under obligations to protect your information;
  • Legal or safety reasons — if required to do so by law, subpoena, or other legal process, or when we believe disclosure is necessary to protect the rights, property, or safety of AC Personal Care, our clients, or others; and
  • Business transfers — if AC Personal Care is ever involved in a merger, acquisition, or sale of assets, website information may be transferred as part of that transaction, subject to this policy.

5. Cookies & Tracking Technologies

As of the effective date above, this website does not set advertising or analytics cookies and does not use third-party tracking pixels. The only outside connections your browser makes when visiting are to load web fonts (Google Fonts) and, if you submit our contact form, to deliver that submission (FormSubmit), as described in Section 2. If this changes in the future — for example, if we add website analytics — we will update this policy to describe what is collected and how to opt out.

6. Data Retention & Security

We retain contact-form submissions only as long as reasonably necessary to respond to your inquiry and for our internal business records, unless a longer retention period is required by law or by our obligations as a Medicaid-participating provider. We use reasonable administrative and technical safeguards designed to protect information submitted through our website; however, no method of transmission over the internet is completely secure, and we cannot guarantee absolute security.

7. Your Privacy Rights

Nevada residents. Nevada law (NRS 603A) gives Nevada consumers the right to submit a request directing an online business not to sell certain categories of covered information the business has collected. AC Personal Care does not sell personal information collected through this website, and has no plans to do so. If you would like to submit a verified request under NRS 603A regardless, you may email us at hello@acpersonalcare.com.

All visitors. You may ask us to tell you what information we hold about you from a website submission, correct it, or delete it, by contacting us using the details in Section 11. We will respond within a reasonable time.

8. Children’s Privacy

Our website is intended for adults seeking or coordinating personal care services and is not directed to children under 13. We do not knowingly collect personal information from children through our website. If you believe a child has provided us information, please contact us and we will delete it.

10. Changes to This Policy

We may update this Privacy Policy from time to time to reflect changes to our website or practices. We will post the revised policy here with an updated "Last Updated" date. Material changes will be noted at the top of this page.
